This clue was in one of the semi-recent free online puzzles:
Twist attendant
The four-letter answer was 'GASP'. Can someone explain this to me? I haven't been able to find any justification for this answer. - ZM

It could be "twist" as in "plot twist". For instance, after a big plot twist in a movie, the expected outburst from the audience is "GASP!" The word "attendant" in the clue would thus denote that GASP usually attends/follows a twist.
I'm not saying it's a particularly good clue (I think "twist reponse" would be better), but I think you could interpret it that way.
